Case Study no.1 - A Client who just wished to Relax
​
History
​
The client is a 27-year-old female. She works in an office, is married and is a mother to a young child. She is quite satisfied with her life and rates her stress levels at work and at home on equal levels, giving them a score of 5 on 10. She has had no significant changes in her life lately, except that her child has started formal schooling. The client doesn’t suffer from any health conditions and visits her doctor for regular check-ups and for that occasional sore throat, headache or gastric discomfort.
During her consultation, she has also reported feeling a need to take more care of herself to be strong enough to care for her family in return. At first she worried this would take away more of her already limited free time, but what she read and heard from her doctor convinced her that some time for herself every day would boost her self-esteem and physical energy to carry out her duties better.
The client chose to reserve an hour a day to do something just for herself and decided to have a one-hour holistic massage once a week. On the rest of the days of the week she enrolled on yoga and meditation, photography and public speaking sessions, together with another two sessions of country walks and jogs during which she listened to her favourite music. She planned to do all this in early mornings or late evening sessions. She also decided to take Wednesday afternoons off to catch up with one of the courses.
Goals
​
The client’s goals were to make some time for herself every day, believing this would in the long run make her happier and more ready to face her daily challenges at work. She also felt that she had neglected her well-being for too long and wanted to invest in something that mattered. The client also believed that this regime would keep her away from negative thoughts and feelings, and thus allow her to manage her moods naturally. She emphasised the importance of keeping as healthy and natural as possible and to use medications and invasive treatments only as a last resort.
Treatment
The client chose home massage because it was convenient for her. She slotted the weekly one-hour holistic massage around her child’s routine. She chose to have her massage every Tuesday afternoon, when her parents usually took her child out for a walk or treat. This enabled her to get ready without interruptions, receive the treatment in the comfort of a quiet room at home without distractions (she always switched off her phone) and spend some extra time taking it easy after treatment to benefit fully from the effects of massage. The latter included keeping well hydrated and spending some quiet time reading, something that she hadn’t found time for before.
Holistic full body massage was carried every week, with the client fully relaxed. She chose to remain quiet throughout most of the treatment, only giving feedback or asking questions when necessary. Soft music was played in the background and the quiet room chosen was kept dimly light. Occasionally it was necessary to work on specific areas of the body more than others, but generally the whole body was treated equally.
Outcome
The client was slightly anxious on the first appointment, but seemed to relax well enough after consultation and when the procedure was explained to her. She relaxed throughout the treatment and only spoke occasionally. Feedback was given following the session. From then on, the client looked forward to each session, always being ready on time. Deep relaxation was possible given the setting of the place and the mindset of the client. The client reported feeling a surreal sense of calm and relaxation in every muscle. Very few modifications were needed from one session to the other and not a session was missed.
The client noted that by the time the family were together, she felt ready to face the evening and slept better each night. In between sessions she also noted that her sleep was deeper and at work she felt calmer and became more aware of her posture and the need to take frequent short breaks. Her walking and jogging sessions also felt lighter following the effect that massage had on her physiology. Her headaches and gastric discomfort eased. The client noticed that she was drinking more water and that she could relax faster when she sat watching television with her husband every evening.
She decided to continue with the weekly treatment indefinitely. She also sounded very satisfied about having successfully completed her photography course and continues to do walking, jogging, yoga and meditation until this day.

Case Study no.2 - A Client with Fibromyalgia
​
History
​
The client is a 45-year-old female. She has suffered body aches and pains and fatigue since she last pregnancy. The client also noticed increased sensitivity in her body especially along knotty lumps, more frequent headaches, sciatica-like pain as well as upper back, shoulder and neck muscle soreness. She had difficulty using full range of motion. She was diagnosed with fibromyalgia around 9 years ago. The client tried getting more rest, exercising more and applying topical ointments, which didn’t do much to alleviate her symptoms. She has since sought the help of 333 massage therapy Malta after getting the go-ahead from her doctor.
​
Goals
The client’s goals are to alleviate the pain which gets worse periodically, especially in the tender points and areas often associated with the condition. Her aim is to raise her quality of life, become more functional in her daily activities and improve her mood.
​
Treatment
​
Following a doctor’s consultation about her condition, the client decided to try massage therapy. Thorough assessment of the client’s needs and expectations was carried out and a massage intake form was filled in. The client was advised to avoid sessions when there was a flare up and also to schedule treatment before potentially stressful situations or events in her life. In the absence of both, she tried to attend regular weekly sessions to maintain the positive results achieved.
​
Treatment included warming up and relaxing the body through initial strokes. This was followed by locating and working on tender and trigger points around the body through adaptive pressure and friction techniques. Finally, full body massage was used to achieve the desired effect of relaxing the whole body. The client was advised to rest, eat healthily and plan her activities wisely to prolong this state of relaxation and flexibility. This was carried out in a quiet room, with dim light and relaxing music to add to the calming atmosphere.
​
The treatment was a welcome alternative to injections and medicine. Apart from being non-invasive, it also proved to be completely natural without the risks associated with treatment with strong medicines.
​
Outcome
​
The above techniques lessened or eliminated pain by the end of the session. The client also reported increased flexibility and range of motion, thus enabling her to function better at work and at leisure. She often described her localised pain and stiffness as ‘disappearing’ instantly and could not believe how some nagging pain had resolved after the session. This positive outcome encouraged her to continue receiving maintenance therapy and seek more specialized sessions when needed.

Case Study no.3 - A Client with Stress and Anxiety
History
​
The client is a 14-year-old male. He has had a very normal childhood and has not been diagnosed with any particular condition or disorder. However, following a few significant changes in his extended family and school routine, he became increasingly stressed and anxious. This started affecting his behaviour at home and at school, especially when faced with new experiences and changes. He was referred by his parent due to his age. The parent chose massage therapy after some research she had carried out on massage therapy, which was found to help issues of attention disorders, psychological problems, gastrointestinal problems and pain syndromes, among others. The parents chose this course of action to try to resolve this issue in a more natural way without the use of medications or further psychological trauma after having discussed the situation with their doctor.
Goals
The client’s goals were to learn to relax, learn to let go and improve his mood. As a result, he hopes to be able to concentrate better at school, deal better with his siblings at home and feel better about himself overall. The client and the parent were informed that this treatment would not involve any psychotherapy and that the therapist was not a psychologist or medical doctor. The parent was required to sign a parental consent form due to the age of the client. The parent also expressed her wish to carry out the treatment in a very casual manner so that her child felt this was a ‘treat’ not some sort of corrective treatment.
Treatment
​
Treatment was carried out in the presence of the parent due to the age of the client. Treatment involved short 20-minute sessions of light to moderate full body massage, focusing mainly on the upper back, shoulders, neck and head. Attention was paid to the client’s level of comfort and tolerance, which changed in every session but stabilised progressively. Soft relaxing music was played during the massage. Treatment was carried out twice a week, with the parent instructed to massage the child at home on the rest of the days. Eventually, the child received massage once a week with the parent delivering sessions at home on alternate days.
Outcome
The client responded positively to these sessions and looked forward to them. It was noted that massage calmed the boy faster and better than quiet time alone. He was observed to be more attentive and less hyperactive at school and had fewer sleep problems by the second week of treatment. The parent also reported noticing fewer sleep disturbances, less anxiety and a better mood into the third week of treatment. Needless to say, anxiety-related gastrointestinal symptoms also resolved. In view of this positive response, after the fourth week the parent continued to massage the child at home three times a week. We look forward for positive feedback from her down the line.

Case Study no.4 - An Elderly Client with Fibromyalgia

History
​
The client is a 71-year-old female. She has suffered various gastrointestinal symptoms, pain all over her body, palpitations, chest pain and fatigue since menopause. The client was concerned about her symptoms of bloating, fatigue and painful knots in her chest and other parts of the body and her high rheumatoid factor bloods result. A myriad of medical tests always came out negative and her rheumatologist eventually diagnosed her with fibromyalgia. She has seen a physiotherapist and performs strengthening exercises at home, which has helped alleviate her symptoms. Since the diagnosis, she has also spent less days in bed feeling sick. She was invited to try out 333 massage’s complementary therapy following medical consent.

Goals
​
The goals were to help the client relax, alleviate the pain in the tender and trigger points and enable her to cope better with her condition.
​
Treatment
​
After assessment of the condition, of the tender point locations and the client’s expectations, massage therapy was provided. The client received a Swedish full body massage to help her relax and warm up the body. This was followed by body work using techniques like myofascial release therapy and positional release techniques. Particular attention was paid to the amount of pressure used, with continuous communication with the client taking place during the session. Live feedback was indispensable to ensure that the right site was being targeted, that the amount of pressure was neither too light nor too hard and that the treatment was producing the expected results. This treatment worked well alongside the medicinal regime that the client was following.

Outcome
​
The client reported feeling more relaxed and pain-free by the end of the session and that she slept better that night. She also described her body as ‘loosened up’ and more flexible. Long term, she felt a reduction in sensitivity to pain in the tender points and felt she could manage and understand her pain better. The fact that she felt better after the one-hour session made her less anxious and more confident in dealing with her condition.

Case Study No.5 - A client who suffered from Body Dysmorphic Disorder

History
​
The client is a 19-year-old female. She studies at MCAST. Apart from that, she has a small circle of friends and doesn’t socialise much due to a condition called Body Dysmorphic Disorder (BDD). BDD is a condition where a person spends a lot of time worrying about flaws in their appearance, which are often unnoticeable to others. Sufferers are oftentimes not satisfied with their bodies or how they look. This can lead to a myriad of other conditions, including eating disorders.
This teenager’s stress levels rose when out of her home, especially in summer. She had often been bullied by fellow students and felt she didn’t fit in. Social media made her condition worse when she couldn’t stop comparing herself to perfectly photoshopped images. She hated her body and felt she would never come to terms with it.
The client got in touch with us after coming across an online post about massage therapy for BDD. It was the hardest decision of her life, but she plugged up courage and sent a message. During her consultation, she confided how embarrassed and ashamed she often felt, which in turn worsened her anxiety. She reported preferring to stay at home and often spent considerable time in front of the mirror, trying to hide her perceived flaws. At other times, she avoided the mirror altogether and compared her appearance with others her age. She even scrutinised the few photos of herself she had on social media and removed most of them. She decided there was no harm in trying out massage therapy when she learned how natural and non-invasive it was.
​
Goals
​
The client’s goals were to create healthy self-connection through therapeutic touch. Since BDD makes you want to distance yourself from your body, she believed massage could bring her back to herself and help her accept and appreciate her body as it is, scars, veins, pigmentation, imperfections and all. She hoped that spending time and money on massage therapy would boost her self-confidence and self-care.
Treatment
​
The client chose to attend the therapist’s wellness centre for privacy reasons. She said she was overwhelmed when the non-judgemental therapist treated her body with respect and care because she hadn’t done that for a long time. The first session was the most difficult, but she persisted and after two or three visits, she started to get used to the procedure and managed to let down her guard.
​
Outcome
​
The client gradually began to feel better about herself and spent less time worrying about her perceived flaws. She felt she could and had a right to feel good about her body and felt more comfortable in the presence of others. Her social life also improved because she now compared herself less to others and felt she could appreciate the body she was in. Part of the healing process involved feeling bad when she read other sufferers’ stories of BDD. Her family and close circle of friends noticed this change and felt a stronger bond. She decided to continue to attend massage therapy every now and then after a set of regular sessions to maintain the positive results.
In a society obsessed with beauty and perfection, massage therapy can send the message that ‘You are perfect, just as you are.’

Case Study No. 6 - A Client who Suffered Pain from Poor Posture
​
History
​
The client is a 43-year-old female teacher. She performs light exercises at home but also spends almost half her time at work sitting at a desk which often leads to a bad posture with hunched shoulders and neck, especially while working at her laptop, during corrections and in the classroom. The client rated her ability to relax as moderate, which also explains the chronically tight back and shoulder muscles. The client often complains of tension and pain in her back, shoulders and neck. She also suffers from sciatic pain, especially when she sits for long hours. Headaches and tired legs are two other lingering symptoms which she often sought medical advice for. Performing house chores oftentimes results in an exacerbation of her symptoms.
Goals
​
The client’s goals were to is to release the tightness and reduce tension and adhesions through Sports Massage techniques, including joint mobilisation, Muscle Energy Techniques, Neuromuscular Techniques, Soft Tissue Release and Connective Tissue Massage These, together with a heightened awareness of good posture and frequent short breaks, are meant to allow the client to assume a more symmetrical and upright posture and reduce the likelihood of this becoming chronic.
Treatment
​
Before the first session, postural assessment was carried out on the client to determine and confirm any postural issues which could be addressed through Sports Massage techniques and strengthening exercises. Treatment was carried out in 1-hour weekly sessions. It involved warming up the areas being worked on and then using specialised techniques to effectively reduce swelling, make the muscles more supple, release trigger points, relieve the pressure that the muscles exerted on the nerves, increase flexibility and range of motion, as well as to stretch and reset particular muscle groups. Some input from the client was required during the treatment (involving contracting and holding muscle stretches) and home advice with regards to hydration and home stretching and strengthening exercises was given after each session.
Outcome
​
The client was afraid the treatment would be painful before it started but felt more relieved after each session. She was grateful for the changes that she felt in her body, especially for how she could perform her daily tasks and training routines pain-free and with more freedom. She also seems to have taken on board my advice for regular training, staying hydrated and adopting a good posture. This client has certainly made a marked difference in her quality of life.

Case Study No.7 - A Young Athlete
​
History
The client is a teenage athlete who plays football four times a week and jogs regularly. Despite his warming up, cooling down and stretching routines, he still suffers from aches and pains in his shins and ITB (side of the thighs). Excessive training and poor techniques could be contributing to his symptoms. The tightness in his legs sometimes makes training difficult and causes him to miss training sometimes.
​
Goals
The client has sought sports massage therapy to help him train better and improve performance. Treatment will focus mainly on the lower body due to the client’s activity. The main goals are two-fold: to release tight bands of muscle and fascia, which will then enable better strengthening of weak areas and restore body mechanics. It was explained to the client that modifications to the treatment plan will be made on a per session basis according to the way his body reacts to the previous session.

Treatment
​
Before the first session, postural assessment was carried out on the client to determine and confirm any postural issues which could be addressed through Sports Massage techniques and strengthening exercises. Treatment was carried out in 1-hour weekly sessions. It involved assessing and extending the range of motion of the feet, knees and legs through connective tissue massage, soft tissue release and muscle energy techniques. In addition, soreness from previous training was addressed through post-event massage. In addition to this, attention was also given to the upper back and shoulders due to the client’s sedentary job. Home advice included demonstrating exercises to strengthen his weak areas and handing out instructions to the client to follow at home.

Outcome
​
The client complied very well and showed a lot of interest in the process. There were times which felt very soothing and relaxing, and others where the client was required to give input or endure some challenging stretches or uncomfortable pressure. Yet, soreness was kept at a minimum and the client always felt better the following day. I felt attuned to the client’s levels of comfort or otherwise through frequent verbal and visual checks. At one point he exclaimed that he had been trying to achieve a specific stretch for a long time and that he had finally felt it through one of my techniques. This made us both very satisfied. Lately he mentioned he could run better following the muscle release treatment and committed himself to attending maintenance sessions fortnightly. This client is living proof that sports massage is not for elite sports people only, but can be used by everyone. The client has grown more confident lately and is assuming more control over his progress. The satisfaction one feels when working with a progressing and grateful client is immense!
